The Burnhams Surgery Publication Scheme

This Publication Scheme provides a guide to
the services and practitioners who practice together within the partnership of Dr. Redman
& Partners at The Burnhams Surgery, Burnham Market as required by the Freedom of
Information Act 2000.
It is based on the model publication scheme
for general practices produced by the NHS Freedom of Information (FOI) project board and
the British Medical Association.
Part One: Introduction
The publication scheme is a complete guide to
the information routinely made available to the public by Drs. Sarah
Gorrod, Marcus Brudenell, Helen Caswell and David Ince. It is a description of the information
about The Burnhams Surgery and the General Practitioners that work there that is available
to the public. It will be reviewed at regular intervals and we will monitor its
effectiveness.
Yours rights to information
The Freedom of Information (FOI) Act 2000 recognises that members of the
public have a right to know how public services are organised and run, how much they cost
and how decisions re made.
From 1 January 2005 the FOI Act will oblige
the Practice to respond to requests about information that it holds, and is recorded in
any format and it will create a right of access to that information. These rights
are subject to some exemptions which have to be taken into account before deciding what
information it can release.
In addition to accessing the information
identified in this Publication Scheme, you are entitled to request information about The
Burnhams Surgery under the NHS Openess Code 1995.
Under the Data Protection Act 1998 you are
also entitled to access your clinical records or any other personal information held abut
you, and you can apply in writing to the Practice Manager at The Burnhams
Surgery to do this.
New environmental information regulations may
be introduced as early as 2003. These will enable similar access to environmental
information as under the Freedom of Information Act 2000.
How much does it cost?
Publications are all free unless otherwise stated. Where there is a
charge for providing information the cost will be calculated as set out in each class.

available?
Information may be downloaded from our Practice website www.burnhammarketsurgery.co.uk otherwise it
is available in hard copy from the Practice Manager as indicated for each class of
information.

Part Two : Classes of Information
All information at The Burnhams Surgery is
held, retained and destroyed in accordance with NHS guidelines.
Our commitment to publish information
excludes any information which can be legitimately withheld under the exemptions set out
in the NHS Openness Code or Freedom of Information Act 2000.
Where individual Classes are subject to
exemptions, the main reasons are the protection of commercial interests and the protection
of confidential personal information under the Data Protection Act 1998. This
applies to all Classes within the Publication Scheme.

This NHS Practice is part of the West Norfolk
Primary Care Trust (PCT); It supplies services in accordance with a general medical
services contract held within West Norfolk PCT.

This Practice aims to follow
national and local best practice guidelines, including NICE best practice guidelines and
National Service Frameworks.
Some information will be withheld including
person, confidential information about individuals that is protected by the Data
Protection Act 1998.
Class 2 Information
Our Services:
A full range of general medical services.
Child health surveillance, minor surgery,
contraceptive services and obstetrics services.
Well woman, healthy heart, diabetes,
asthma, COPD, ECG and leg ulcer clinics.
Dietetic clinic, once very 2 weeks provided
by the Queen Elizabeth Hospital community dietetic service
Ante natal clinic every Tuesday afternoon
provided by the Iceni community midwifery team based at the Queen Elizabeth Hospital.
Counselling clinic every Friday provided by
our trained counsellor.
A support service provided by the primary
care mental health support worker.
Some services require information with other
health care agencies and we follow the Norfolk NHS & Social Care Information Community
protocol for the sharing of personal information. A copy is available for perusal at
the Surgery; please ask at Reception.

Out of hours cover is provided
by the Fakenham Co-operative group of GPs.
Co-operative between the hours of 7.00pm and
7.00am on weekdays and after 1.00pm on Saturdays and all day on Sundays and Bank Holidays.
The out of hours service is accessed via
Medicom (the Norfolk-wide medical answering service) on telephone number 01603 488488.
Members of staff conduct consultations in
English. If translation services are required please contact us in advance.
Interests:
Dr. Sarah Gorrod has an interest in Asthma, the Cardiac Clinic and Family Planning
and is lead partner.
Dr. Marcus Brudenell has an interest in Minor Surgery and Men's Health.
Dr. Helen Caswell has an interest in Children and Maternity, Contraception and
Minor Surgery.
Dr. David Ince has an interest in diabetes.
Financial and Funding:
The Burnhams Surgery receives money from Norfolk PCT according to its contract with West Norfolk PCT in exchange for services
provided for patients.

nominal charge to cover costs if you require a hard copy of information.
These charges will vary according to how information is
made available.
